The App Well Interprofessional Clinic (IPC) offers assessments and referrals to help older adults in the community improve and/or maintain their health and well-being as they age.

Age-Based Health Assessments
Annual assessments are available by appointment. Appointments are typically brief and can include assessments for:
- Balance and walking
- Depression and anxiety
- Memory and recall
- Nutrition and diet
- Quality of life measures
Additional assessments and/or referrals are made as needed. Information can be shared with primary care physicians with participant permission.
